Application Forms

Create, edit, open, close, and delete application forms from the Dashboard or Discord.

An application form contains its name, questions, log channel, access rules, and actions after a decision. Create a separate form for each purpose so applicants and reviewers know what they are handling.

Create a form

Create a form from the DashboardUse the full-page editor to review the complete form before saving.
  1. Open Dashboard → Applications → Forms.
  2. Select Create Form.
  3. Enter a clear name and optional description.
  4. Choose a private log channel.
  5. Add at least one question.
  6. Save the form, then set it to open.

  1. Run /application settings.
  2. Select Add new form.
  3. Open Edit Info and add a name, description, and log channel.
  4. Open Questions and add at least one question.
  5. Return to the main form menu and set the form to open.

Name, description, and log channel

Name appears in the form picker and review screens. It must contain 1 to 100 characters. Use a specific name such as Moderator Application.

Description tells members what the form is for. It is optional and supports up to 1,024 characters.

Log Channel receives completed answers. Use a private staff channel that Discortics can view and send messages in. It is required on the public bot and recommended on Prime and Custom bots.

Open or close a form

  • Open forms appear when a member runs /application apply.
  • Closed forms keep their questions and responses but cannot receive a new application.

Close a form when recruitment ends temporarily. Delete it only when you no longer need its configuration.

Edit a form

Edit from the DashboardDashboard edits are applied after you use the save bar.
  1. Open Dashboard → Applications → Forms.
  2. Select the form.
  3. Change its information, questions, restrictions, or actions.
  4. Use the save bar before leaving.

Edit from DiscordOpen the existing form from the settings menu.
  1. Run /application settings.
  2. Select the form.
  3. Choose the setting you want to change.
  4. Complete the menu or modal. The change saves immediately.

Delete a form

Open the form and select Delete, then confirm. Deleting a form removes its configuration permanently.

Deleting the form does not clear its stored responses. Use /application delete first when you also need to remove its response history. If you only want to stop new applications, close the form instead.

Warning: A deleted form cannot be restored. Record its questions and settings before confirming if you may need them later.

Form checklist

Before sharing a form, confirm that:

  • Its name clearly describes the application.
  • It has at least one question.
  • The log channel is private and accessible to Discortics.
  • Its status is open.
  • Its role and channel restrictions allow the intended members.
  • A non-staff test account can complete it in DMs.
